OBJECTIVE The purpose of this prospective, randomized, blinded study was to determine the effect of trephination on postoperative pain and swelling in symptomatic necrotic teeth. STUDY DESIGN Fifty emergency patients participated, and each had a clinical diagnosis of a symptomatic necrotic tooth with associated periapical radiolucency. After endodontic treatment, patients randomly received either a trephination or mock trephination procedure. The trephination procedure used an intraosseous…
Effect of apical trephination on postoperative pain and swelling in symptomatic necrotic teeth.
TLDR
Because there was not a significant reduction in pain, percussion pain, swelling, or swelling the authors cannot recommend the routine use of an apical trephination procedure, as used in this study, in symptomatic necrotic teeth with radiolucencies.
Effect of drainage upon access on postoperative endodontic pain and swelling in symptomatic necrotic teeth.
TLDR
Obtaining short-term drainage upon access did not significantly reduce pain, percussion pain, swelling, or the number of analgesic medications taken for symptomatic necrotic teeth with periapical radiolucencies.
Preoperative pain and medications used in emergency patients with irreversible acute pulpitis or acute apical periodontitis: a prospective comparative study.
TLDR
Pain to percussion and palpation, lymphadenopathies, and dental mobility were strong indicators for IAP and self-medication offered better relief in cases of IAP than in Cases of AAP.
Clinical strategies for managing endodontic pain
TLDR
This article will review procedures used to treat endodontic emergencies in the context of relevant controlled clinical trials and their underlying biological principles.
COMPARISON OF THE EFFECTS OF BUPIVACAINE AND LIDOCAINE AS LOCAL ANESTHETICS ON THE INCIDENCE OF PAIN AFTER ROOT CANAL THERAPY
TLDR
Long acting local anesthetics can be used in endodontic treatment especially for patients with high prediction of postoperative pain, as shown in this double blind clinical trial study.
Interappointment pain: mechanisms, diagnosis, and treatment
TLDR
This review focuses on the mechanisms of interappointment pain, with special emphasis placed on the causative agents and the host response to injury that can precipitate pain.

Factors associated with endodontic flare-ups: a prospective study.
TLDR
Flare-ups were found to be positively correlated with multiple appointments, retreatment cases, periradicular pain prior to treatment, presence of radiolucent lesions, and patients taking analgesic or anti-inflammatory drugs, while there was no correlation between flare-up, and age, sex, different arch/tooth groups and the status of the pulp.
