TLDR
"swimmer's shoulder" is not an anatomically based diagnosis and that the frequent lack of a specific diagnosis probably has hindered treatment, but new investigative tools, such as arthrography and NMR imaging, have improved the chances of reaching an accurate diagnosis.
Cerumen impaction.

Depending on available equipment, physician skill, and patient circumstances, treatment options for cerumen impaction include watchful waiting, manual removal, the use of ceruminolytic agents, and irrigation with or without ceruminoslytic pretreatment.

TLDR
Thorough cleansing of the canal is essential for diagnosis and treatment, but flushing should be avoided and acidification with a topical solution of 2 percent acetic acid combined with hydrocortisone for inflammation is effective treatment in most cases and, when used after exposure to moisture, is an excellent prophylactic.
