This review considers whether coronary artery bypass grafting (CABG) performed on the beating heart (off-pump or OPCAB) will supersede conventional CABG utilizing cardiopulmonary bypass (CPB) and cardioplegic arrest as the accepted gold standard. Randomized controlled trials, case-matched reports and observational studies have demonstrated lower morbidity in off-pump compared to conventional on-pump CABG with equivalent mid-term outcome at a significantly lower cost.
