Background: Kratom (Mitragyna speciosa Korth) use has increased in Western countries, with a rising number of associated deaths. There is growing debate about the involvement of kratom in these events. Aims: This study details the characteristics of such fatalities and provides a ‘state-of-the-art’ review. Methods: UK cases were identified from mortality registers by searching with the terms ‘kratom’, ‘mitragynine’, etc. Databases and online media were searched using these terms and ‘death…

Figures and Tables from this paper

Prevalence and description of kratom (Mitragyna speciosa) use in the United States: A cross-sectional study.
TLDR
Estimated United States past-year prevalence of kratom use is 0.8%, and kratom users tend to have more serious substance abuse profiles than non-users or users of cannabis, alcohol, or cigarettes, the first description of k Mitragyna speciosa use at the national level.
The Causes of Death and Pathological Findings of Kratom Users
TLDR
The use of mitragynine may be a direct or indirect cause of death, whereas the lethal concentration has yet to be clearly determined.
The Adverse Cardiovascular Effects and Cardiotoxicity of Kratom (Mitragyna speciosa Korth.): A Comprehensive Review
TLDR
Although kratom use has been associated with death and cardiotoxicity, especially at higher doses and when associated with other psychoactive drugs, the dearth of data and methodological limitations reported in existing studies do not allow a definitive conclusion, and further studies are still necessary to address this issue.
Assessment of Cardiovascular Functioning Among Regular Kratom (Mitragyna speciosa Korth) Users: A Case Series
TLDR
Regular kratom use without concomitant use of other illicit substances may not provoke any risk of cardiovascular impairment or toxicity except for prolonged QTc interval, which appeared to be dose dependent.
A comparative analysis of kratom exposure cases in Thailand and the United States from 2010-2017
TLDR
Despite lower frequencies of co-ingestants overall, US kratom abuse exposures yielded greater clinical severity, and this disparity may be attributable to differences in the products labeled “kratom,” greater sedative co-exposures in the US, and/or differences in population genetics or use patterns.
Is kratom (Mitragyna speciosa Korth.) use associated with ECG abnormalities? Electrocardiogram comparisons between regular kratom users and controls
TLDR
There was no link between regular kratom use and electrocardiographic abnormalities with an estimated average daily intake of 434.28 mg of mitragynine, and there were no differences in the odds of having prolonged QTc intervals between kratom users and controls.
Lipid profile of regular kratom (Mitragyna speciosa Korth.) users in the community setting
TLDR
Regular kratom consumption was not linked to elevated serum lipids, except when there is a higher frequency of daily kratom intake, according to the findings of this analytical cross-sectional study.
Current and Future Potential Impact of COVID-19 on Kratom (Mitragyna speciosa Korth.) Supply and Use
TLDR
A shortage in the supply of kratom products may place unexpected burdens on kratom users, potentially influencing some who use kratom for SUD self-treatment to regress to harmful drug use, hence increasing the likelihood of adverse outcomes, including overdose.

Characterization of urinary protein profile in regular kratom (Mitragyna speciosa korth.) users in Malaysia
TLDR
The preliminary findings indicate that regular consumption of freshly brewed kratom solution over a protracted period seems to induce proteinuria, suggestive of an early stage of kidney injury, and further studies are urgently needed to confirm the findings, and establish kratom’s renal impairing effects.

Kratom Abuse in Ramathibodi Poison Center, Thailand: A Five-Year Experience
TLDR
A retrospective review of Kratom exposure cases from the RPC toxic surveillance system found most cases in both groups had good prognostic outcome, and there was a baby with withdrawal symptoms who was delivered from a chronic Kratom-abusing mother, suggesting possible exposure via the transplacental route.
Notes from the Field: Kratom (Mitragyna speciosa) Exposures Reported to Poison Centers - United States, 2010-2015.

TLDR
This report highlights the current scientific and legal controversies regarding kratom and its mitragynine constituents, which prompted a massive response from kratom advocates.
Mitragynine 'Kratom' related fatality: a case report with postmortem concentrations.
TLDR
A 24-year-old man whose medical history was significant for alcohol abuse and depression was found unresponsive in bed and postmortem findings were unremarkable apart from pulmonary edema and congestion, and urinary retention, and the lack of potential for mitragynine postmortem redistribution.
A Case Report of Inpatient Detoxification after Kratom (Mitragyna speciosa) Dependence
TLDR
These observations support the idea that kratom dependence syndrome is due to short-acting opioid receptor agonist activity, and suggest that dihydrocodeine and lofexidine are effective in supporting detoxification.
Histologic Characterization of Kratom Use-Associated Liver Injury
TLDR
This is the first case report detailing the histology of kratom use-associated liver injury and the patient improved clinically and biochemically and was discharged 8 days after the initial presentation.
