We report a case of CD3‐negative, CD20‐positive T‐cell prolymphocytic leukemia (T‐PLL). The leukemic cells were of medium‐to‐large size, mature‐looking, and did not have cytoplasmic granules. The leukemic cells were negative for surface CD3, CD2, and CD7 and strongly positive for CD20. T‐cell lineage markers such as CD4, CD5, and cytoplasmic CD3 were also positive. A monoclonal rearrangement of the T‐cell receptor (TCR) β chain gene was detected. CD3‐negative T‐PLL has been reported often, but…

The spectrum of morphologic, phenotypic, and clinical features of B-cell neoplasms aberrantly expressing CD3 are expanded, and DLBCLs with anaplastic features and the single follicular lymphoma coexpressed multiple T-cell antigens in a predominantly membranous pattern and presented with nodal disease in a relatively younger patient population.

Phenotypically, the aberrant cell populace demonstrated a CD4+, CD7−, and CD62L− phenotype; at variance with classic MF was the expression of CD20, and although there were a few PAX5-positive staining cells, definitive colocalization studies were negative.
T-cell prolymphocytic leukemia in Japan: is it a variant?
- MedicineInternational Journal of Hematology

It is proposed that Japanese cases of T-PLL may constitute a variant, based on clinical and biological features of 13 cases diagnosed between 1992 and 2009 in the Tohoku region of Japan, which were compared with three Western series.

Early detection of systemic symptoms is very important in the management of solitary plasmacytoma and high proliferation index (Ki67>8%) and tumor size (>5cm) were the risk factors for rapid progression of plasmatetoma.
