BACKGROUND AND PURPOSE Bilateral vertebrobasilar junction agenesis is an exceptional anatomic variation. This article explores the angiographic characteristics of this variant and its embryologic mechanisms. MATERIALS AND METHODS Two observations of bilateral agenesis of the vertebrobasilar junction are reported.
