}BACKGROUND
To examine the efficacy of early versus late spinal fracture fixation, we reviewed National Trauma Data Bank (NTDB) records to identify the breakpoint in reported timing of operative fixation. Using this breakpoint we then analyzed outcome for those treated early versus late, hypothesizing that the early group would experience better outcome as reflected by resource utilization and complications.
METHODS
The NTDB was queried for patients with any level spinal fracture that required…

In patients with isolated spinal trauma, time until spinal operation does not seem to be an important predictor of functional outcome at the time of hospital discharge, whereas treatment at Level I trauma centers seemed to confer marginally better outcomes.

The EAC recommendations are safe and effective for the majority of severely injured patients with mechanically unstable femur, pelvis, acetabular, or spine fractures requiring fixation and are associated with fewer complications and shorter LOS.

Patients with open tibial fracture treated at hospitals with more timely initiation of surgical care had lower cost and shorter length of stay than patients treated at hospital with less timely initiated of care, and policies directed toward mitigating variation in care may reduce unnecessary waste.
Timing of Surgery for Thoracolumbar Spine Trauma: Patients With Neurological Injury.
- MedicineClinical spine surgery

Patients with thoracolumbar fracture and associated neurological injury who underwent surgery within 3 days of admission experienced fewer in-hospital complications and may be due to secondary injury mechanism avoidance and earlier mobilization.
Does Early Fracture Fixation of Thoracolumbar Spine Fractures Decrease Morbidity or Mortality?

These studies demonstrated that early stabilization of thoracolumbar fractures reduced the mean number of days on a ventilator, the number of Days in intensive care unit and in hospital, and reduced respiratory morbidity compared with late stabilization.
Complications in acute phase hospitalization of traumatic spinal cord injury: Does surgical timing matter?
- MedicineThe journal of trauma and acute care surgery
- 2013
This study showed that a shorter surgical delay after a traumatic SCI decreases the rate of complications during the acute phase hospitalization, and suggests that patients with traumaticSCI should be promptly operated on earlier than 24 hours following the injury to reduce complications while optimizing neurologic recovery.
