}SUMMARY In April 2005, an outbreak of Chikungunya fever occurred on the island of Réunion in the Indian Ocean. During winter 2005, six patients developed meningoencephalitis and acute hepatitis due to Chikungunya virus. Our objectives were to determine the incidence and mortality of atypical Chikungunya viral infections and to identify risk factors for severe disease. A hospital-based surveillance system was established to collect data on atypical Chikungunya cases. Case reports, medical…

During a CHIK outbreak in a DVI-endemic area, overdiagnosis of CHIK was common and skin rash during fever and white blood cell count ≥ 5000 cells/mm3 or specific antigen testing (if available) can be helpful in differentiating CHIK from DVI.

It is observed that CHIKV intra-host genetic diversity increased over time and that a higher viral amino-acid complexity at the acute stage was associated with increased number of arthralgia and intensity of sequelae at day 300.

The number of imported CHIKV cases in all of Italy in 2006 was likely higher than previously thought, and most patients probably did not seek medical care, and when they did, physicians may have failed to recognize the disease.

Yet another challenge faced during this large outbreak in the country has been the lack of rapid diagnostic facilities, which probably suggest that there is indeed lack of herd immunity to chikungunya virus.
