Patients with major depression consistently show hyperactivity of the hypothalamic-pituitary-adrenocortical (HPA) axis. Successful antidepressant drug therapy reverses the HPA system abnormalities, but the clinical improvement of depression occurs in weeks rather than days. These improvements do not correlate with changes in monoaminergic neurotransmitters but coincide with the time course of antidepressant actions on glucocorticoid receptors (GRs).
