Autoimmune encephalitis with antibodies to NMDA receptors (anti-NMDAR encephalitis) is a frequent form of autoimmune encephalitis. The disease is curable, though the lack of timely treatment can lead to disability in patients or death. Difficulties in the diagnosis of anti-NMDAR encephalitis arise from its heterogenous manifestations, its possible combination with other autoimmune diseases, and lack of doctors’ awareness of this type of encephalitis. This article reviews published data and…

Recent studies have clearly documented that both clinical manifestations and the antibodies may contribute to early diagnosis and multidisciplinary care in anti-NMDAR encephalitis, and clinicians should be aware of the overlapping diseases.

Findings indicate that “juvenile acute nonherpetic encephalitis” or a subset of this disorder is mediated by an antibody-associated immune response against NR1/NR2 heteromers of the NMDA receptor (NMDAR), and the severity and extended duration of symptoms support tumor removal.

A 15-year-old girl who initially presented with encephalopathy, hypoventilation, dyskinesias, and seizures is described, which was atypical, with more than 10 relapses during the next year, with longitudinally extensive transverse myelitis and optic neuritis in addition to multifocal, contrast-enhancing gray and white matter lesions.

Results of this case study suggest that the development of autoantibodies against NMDA-type glutamate receptors may play a role in the pathogenesis of central nervous system demyelinating diseases.

The authors report here 2 cases of subacute-onset encephalitis with N-methyl-D-aspartate (NMDA) receptor antibodies, one of the reported cases is the first known case of paraneoplastic encephalopathy with NMDA antibodies in a child with neuroblastoma, and this report suggests that the spectrum of symptoms of enphalitis with NMda receptor antibodies is probably wider than previously thought.
