An Update on the Pathophysiology of Complex Regional Pain Syndrome
@article{Bruehl2010AnUO, title={An Update on the Pathophysiology of Complex Regional Pain Syndrome}, author={Stephen Bruehl}, journal={Anesthesiology}, year={2010}, volume={113}, pages={713-725} }
Complex regional pain syndrome (CRPS) is a neuropathic pain disorder with significant autonomic features. Few treatments have proven effective, in part, because of a historically poor understanding of the mechanisms underlying the disorder. CRPS research largely conducted during the past decade has substantially increased knowledge regarding its pathophysiologic mechanisms, indicating that they are multifactorial. Both peripheral and central nervous system mechanisms are involved. These include…

Mechanism-based treatment in complex regional pain syndromes
- Medicine, PsychologyNature Reviews Neurology
- 2014
The challenges in linking the current theories and knowledge of pathophysiological mechanisms to the mode of actions of the different treatment approaches are described, and the concept of mechanism-based treatment for CRPS is established.
Review of complex regional pain syndrome and the role of the neuroimmune axis
- MedicineMolecular pain

Preliminary neurostimulation findings seem to provide a compelling rationale for its use in CRPS pain treatment, and increasing evidence supports a role for inflammation and neuroinflammation in the pathophysiology of CRPS.

This topical review gives a synopsis of CRPS I and discusses the current concepts of the authors' understanding ofCRPS I in adults, the diagnosis, and treatment options based on the limited evidence found in medical literature.

Findings suggest there may be factors unique to pediatrics due to developmental changes in neuroplasticity as well as somatic, endocrinological and emotional growth that may shed light on the adult condition.
Complex regional pain syndrome: more than a peripheral disease.

A more widespread and bilateral pattern of CNS reorganization appears to characterize CRPS, which might be related to dysfunctions in the basal ganglia or in thalamo-cortical structures, and treatment should involve not only anti-inflammatory measures and pain therapy, but also the integration of neurorehabilitative training programs.
Complex Regional Pain Syndrome: An inflammatory disease
- Medicine, Biology

Some immunological aspects were further explored with the aim of achieving more insight in both the pathophysiology and possible treatment options in CRPS and showed a trend towards a greater reduction of TNF-α in the intervention group compared with the placebo group.
Complex Regional Pain Syndrome: An update
- MedicineMediterranean journal of rheumatology
- 2019
This syndrome develops in two phases, the acute (warm) phase, with the classic symptoms of inflammation, and the chronic phase, often characterized by trophic changes of the soft tissues and even bones, which is the dominating feature.

Increased systemic CGRP levels in patients with acute CRPS suggest neurogenic inflammation as a pathophysiologic mechanism contributing to vasodilation, edema, and increased sweating, however, pain and hyperalgesia, in particular in chronic stages, were independent of increased neuropeptide concentration.

Elevated IL-8 and sTNFR I/II levels indicate an association between CRPS I and an inflammatory process, and the hypothesis of neurogenic-induced inflammation mediated by neuropeptides is supported by elevated substance P levels.
