}BACKGROUND
Alcohol consumption may be linked to syncopal events. The mechanisms by which alcohol may induce syncope are unknown. Impairment of the response to orthostatic stress may be involved. Using a double-blind, randomized, placebo-controlled study, we tested the hypothesis that short-term alcohol intake causes orthostatic hypotension because of an impairment in the vasoconstrictor response to orthostatic stress.
METHODS AND RESULTS
We examined the effects of alcohol on blood pressure…

The lack of MSNA adjustment to a larger drop in arterial blood pressure during progressive LBNP, coupled with altered sympathetic burst latency responses, suggests that alcohol blunts MSNA responses to orthostatic stress.

Pressor responses to acute alcohol consumption are potentiated in hypertensive patients compared with normotensive controls, and Vasoconstrictor sympathetic tone is not suppressed in hypertension patients after alcohol, despite the enhanced pressor response.

Despite lower BrAC, ingestion of alcopops induced acute vasodilation and hypotension in sitting position and an encroach of the hemodynamic reserve during active standing, which could be of clinical importance with higher doses of alcohol or if combined to other hypotensive challenges.

In the conventional HUT protocols, including ISP provocation, it was difficult to produce a positive response in the patients with alcohol-related syncope, but alcohol ingestion was a useful diagnostic provocation method in such patients.

Eating CF induced a greater cardiometabolic load suggesting that hemodynamic reserves have been encroached during active standing, which may impede the cardiovascular capacity during physical exercise or stress situations, particularly in elderly subjects who are at greater risk for postprandial hypotension and cardiovascular diseases.

Baroreceptor dysfunction causes wide swings in blood pressure that are unrelated to posture, whereas a variety of stimuli may precipitate paroxysmal parasympathetic activation, and sympathetic orthostatic hypotension associated with hypovolemia may occur with mitral valve prolapse or as an idiopathic condition.
