}BACKGROUND
Adverse events related to drugs occur frequently among inpatients, and many of these events are preventable. However, few data are available on adverse drug events among outpatients. We conducted a study to determine the rates, types, severity, and preventability of such events among outpatients and to identify preventive strategies.
METHODS
We performed a prospective cohort study, including a survey of patients and a chart review, at four adult primary care practices in Boston…

Pediatric Adverse Drug Events in the Outpatient Setting: An 11-Year National Analysis
- MedicinePediatrics
- 2009
The national incidence of pediatric ADEs requiring medical treatment, the pediatric population seeking care for ADEs, and the events in terms of patient symptoms and medications implicated vary by age indicate that age-specific approaches for monitoring and preventing ADEs may be most effective.
Prevalence of Adverse Drug Events in Ambulatory Care: A Systematic Review
- Medicine, Political ScienceThe Annals of pharmacotherapy
- 2011
Despite a recent increase in publications on ADEs in the ambulatory care setting, most studies remain hospital based, and notable differences in prevalence rates by age groups and by responsible drug categories provide guidance on how to direct attention toward effective targets for improvement of medication safety in ambulanceatory care settings.
Adverse Drug Events in Adult Patients Leading to Emergency Department Visits
- MedicineThe Annals of pharmacotherapy
- 2010
Adult ADE-related ED visits are frequent in Newfoundland and Labrador, and in many cases are preventable, so further efforts are needed to reduce the occurrence of preventable ADEs leading to ED visits.
Ambulatory care adverse events and preventable adverse events leading to a hospital admission
- Medicine, Political ScienceQuality and Safety in Health Care
- 2007
An estimated 75 000 hospitalisations per year are due to preventable adverse events that occur in outpatient settings in the US, resulting in 4839 serious permanent injuries and 2587 deaths.
Characteristics of ambulatory anticoagulant adverse drug events: a descriptive study
- MedicineThrombosis journal
- 2010
Ambulatory warfarin-related ADEs have significant patient and healthcare utilization consequences in the form of bleeding events and associated hospital admissions, and recommendations for improvement in anticoagulation management include use of information technology to assist monitoring and follow-up documentation.

Assessment of adverse drug events among patients in a tertiary care medical center.
- MedicineAmerican journal of health-system pharmacy : AJHP : official journal of the American Society of Health-System Pharmacists
- 2006
Certain age groups, diagnoses, admission sources, types of insurance, and the use of specific medications or medication classes were associated with increased AE rates at a tertiary care medical center.
Identifying Adverse Drug Events in Older Community-Dwelling Patients
- MedicineThe Annals of Family Medicine
- 2019
The patient-report instrument for identifying adverse drug events (ADEs) in older populations with multimorbidity in the community setting demonstrated good predictive value and could be used to differentiate between symptoms of ADEs and chronic disease in thecommunity setting.
Emergency Department Visits Caused by Adverse Drug Events
- Medicine
- 2007
ADEs leading to emergency department visits are frequent, and many are preventable, confirming that there is a need to develop prevention strategies.
Ambulatory Computerized Prescribing and Preventable Adverse Drug Events
- MedicineJournal of patient safety
- 2016
The implementation of computerized prescribing in the ambulatory setting was not associated with any change in preventable ADEs but was associated with a decrease in potential ADEs at Indianapolis but an increase at Boston, although the absolute rate of ADEs was much lower in Boston.

The high proportion that are due to management errors suggests that many others are potentially preventable now, and reducing the incidence of these events will require identifying their causes and developing methods to prevent error or reduce its effects.
