}BACKGROUND
Blockade of the renin-angiotensin-aldosterone system in the early post-transplant period remains controversial. Angiotensin II-receptor blockers (ARB) have many benefits to the patient with chronic kidney disease and these benefits may also apply to the renal transplant recipient (RTR). Additionally, there are theoretical benefits of ARB use in RTR. This study was designed to investigate the safety of early ARB use after renal transplantation.
METHODS
RTR with serum creatinine…

ACE-inhibitor or AT2-antagonist therapy of renal transplant recipients is associated with an increase in serum potassium concentrations.

Serum potassium levels were compared over an up to 10 years of observation period between subjects with versus without ACEI/ARB therapy using a mixed effects general linear model and the hyperkalaemic effect of ACEi/ARB is balanced by the use of diuretics.

Safety and efficacy of administering the maximal dose of candesartan in renal transplant recipients
- MedicineClinical and Experimental Nephrology
- 2011
The impact of maximal renin−angiotensin system (RAS) blockade on both proteinuria and albuminuria is demonstrated, which indicates the need for future, long-term randomized prospective trials to further establish the impact ofmaximal RAS blockade on renal and cardiovascular protection in transplant patients.
Determinants and patterns of renin–angiotensin system inhibitors’ prescription in the first year following kidney transplantation
- MedicineClinical transplantation
- 2008
Determinants of ACEi/ARB prescription were identified among various recipient characteristics (at transplantation and during follow‐up), donor characteristics and transplant parameters, and nephrologists were more likely to prescribe ACEi /ARB when the transplant parameters were favorable.

The increase in serum potassium level after ARB initiation makes it necessary to monitor serum potassium levels continuously during ARB treatment; however, the risk of hyperkalaemia appeared to be similar for ARB and CCB treatments.

Administration of amlodipine significantly increased the plasma concentration of ciclosporin, and its effects on BP control and cic Losporin concentration may be associated with the CYP3A5 genotype in these subjects.
The renin angiotensin system blockade in kidney transplantation: pros and cons
- MedicineTransplant international : official journal of the European Society for Organ Transplantation
- 2008
There is lack of evidence for supporting that RAS blockade improves neither graft nor patient survival in comparison with other antihypertensive drugs, and Randomized, prospective, double blind, placebo‐controlled trials with enough sample size and follow-up are needed to address the potential role of Ras blockade.
