To investigate the role of extracorporeal detoxification in cirrhotic patients with advanced hepatic encephalopathy not responding to medical treatment, 20 patients were randomized to receive six hours of additional sorbent dialysis or ongoing standardized medical treatment. Following treatment, the clinical stage of encephalopathy remained unchanged in both groups. Abnormal sensory evoked potentials improved following sorbent dialysis (N70 latency, 128 ms before versus 110 ms after treatment…
Effects of Sorbent Suspension Dialysis on Plasma Amino Acid Levels in Cirrhotic Patients with Refractory Hepatic Encephalopathy
In cirrhotic patients, plasma amino acid levels are severely deranged. A decreased ratio of branched-chain to aromatic amino acids (Fischer ratio) has been implicated in the pathogenesis of hepatic
Extracorporeal support of the failing liver
TLDR
Current knowledge suggests that extracorporeal liver support can successfully stabilize liver function, improve the clinical condition of patients, and considerably improve survival in certain subgroups of patients with fulminant hepatic failure and acute decompensation of chronic liver failure.
Extracorporeal Treatment in Fulminant Hepatic Failure: Pathophysiologic Considerations
TLDR
The pathophysiology of hepatic encephalopathy and cerebral edema is integrated into a proposed future concept of liver support for patients on the waiting list for liver transplantation.
Improvement in Central Nervous System Functions During Treatment of Liver Failure with Albumin Dialysis MARS—A Review of Clinical, Biochemical, and Electrophysiological Data
TLDR
In conclusion, MARS significantly improves central nervous system functions and can serve as a model for the further investigation of the role of protein-bound substances in hepatic encephalopathy and cerebral hemodynamics.
Influence of Hydroxy Ethyl Starch Infusion on Serum Bilirubin Levels in Cirrhotic Patients Treated with Artificial Liver Support
TLDR
Hydroxy ethyl starch solution may decrease bilirubin levels in hyperbilirub inemic cirrhotic patients receiving extracorporeal detoxification using a liver support system (FPSA).
Performance of the Hepatic Encephalopathy Scoring Algorithm in a Clinical Trial of Patients With Cirrhosis and Severe Hepatic Encephalopathy
TLDR
The traditional West Haven criteria is modified to provide an objective assessment of the cognitive parameters to complement the subjective clinical ratings for the performance of extracorporeal albumin dialysis using a molecular adsorption recirculating system in patients with cirrhosis and severe encephalopathy.
Citrate pharmacokinetics and metabolism in cirrhotic and noncirrhotic critically ill patients
TLDR
This first systematic study on citrate pharmacokinetics and metabolism in critically ill patients confirms a major role of hepatic citrate metabolism by demonstrating reduced citrate clearance in cirrhotic patients.
The artificial liver — Liver support systems
TLDR
A variety of supportive therapies for patients with acute liver failure have been proposed, including charcoal haemoperfusion, haemodialysis, plasmapheresis,ex vivo liver perfusion and cross circulation, but none have proved efficacious when studied in carefully controlled trials.
Sistemas de soporte heptico artificial. Revisin de la bibliografa mdica
TLDR
Evidence showed that albumin dialysis based systems were able to reduce toxic parameters and improve clinical results in these settings, however, more studies are needed to further evaluate survival and adverse events based on standardised notification.
Evaluation of mental state in a clinical trial of MARS for patients with acute hepatic encephalopathy: comparison of two scales
TLDR
The problem of how HE should be assessed, together with the considerable diversity of terms used to define different clinical settings, prompted a gathering of a panel of international experts to reach a consensus.

Clinical effects of a sorbent suspension dialysis system in treatment of hepatic coma (the BioLogic-DT).
TLDR
The neurologic improvement of patients with acute deterioration of liver function, high serum ammonium, and an average coma level indicates that many toxins of hepatic failure are dialyzable across cellulosic membranes and bound by charcoal.
Biocompatibility of a cuprophane charcoal-based detoxification device in cirrhotic patients with hepatic encephalopathy.

TLDR
Despite technical refinements compared with charcoal hemoperfusion,Biocompatibility of sorbent suspension dialysis is still very limited and it is suggested further developments in design and appropriate strategies of anticoagulation to improve the biocompatible of artificial liver support.
Temporary Extracorporeal Liver Support for Severe Acute Alcoholic Hepatitis Using the Biologic-DT
TLDR
The further development of the BioLogic-DT dialyser with the addition of a plasma treatment module capable of removing cytokines would be worth evaluating in acute alcoholic hepatitis.
Evaluation of the BioLogic-DT sorbent-suspension dialyser in patients with fulminant hepatic failure
TLDR
Removal of metabolites by treatment was limited, with no significant effect on blood ammonia level and further developments of the system will be needed for this very sick group of patients.
Double-blind randomized clinical trial comparing neomycin and placebo in the treatment of exogenous hepatic encephalopathy.
TLDR
The group of excluded patients (n = 63) was compared with the randomized group, and no statistical differences were found regarding sex and age distributions, Child-Pugh classification, etiology of cirrhosis, percipitating factors and grade of hepatic encephalopathy.
Controlled trials of charcoal hemoperfusion and prognostic factors in fulminant hepatic failure.
TLDR
There was in both trials a significant relationship between survival and etiology quite independent of the use or duration of hemoperfusion, except in the non-A, non-B hepatitis and halothane or drug reaction subgroups, which had a high mortality throughout.
Hemodiabsorption in treatment of acute hepatic failure and chronic cirrhosis with ascites.

TLDR
Improvements including addition of osmotically active agents to the priming fluid and sorbent to increase blood oncotic pressure and transfer of ascitic fluid to blood while ultrafiltration transfers fluid out of the blood are now beginning in patients with ascites and encephalopathy.
IMPROVEMENT OF HEPATIC ENCEPHALOPATHY TREATED WITH FLUMAZENIL
TLDR
Flumazenil may be valuable in the treatment of HE in acute and chronic liver failure and in 5 of the 8 episodes that did not respond to flumazanil patients had clinical evidence of brain oedema.
Flumazenil in cirrhotic patients in hepatic coma: A randomized double‐blind placebo‐controlled crossover trial
TLDR
It is concluded that flumazenil is effective in a subset of highly selected cirrhotic patients with severe hepatic encephalopathy and that this beneficial effect is not related to the presence of benzodiazepines in the blood.
mechanism of the excessive sedative response of cirrhotics to benzodiazepines: Model experiments with triazolam
TLDR
It is concluded that, in patients with cirrhosis, disproportional sedation after benzodiazepine administration may be due not only to impaired drug elimination, but also to hypersensitivity of the brain.
