34歳,男性。発熱,四肢の筋肉痛を訴え受診。血清可溶性IL-2受容体,フェリチン高値,PETでは全身皮下に結節状集積が多発した。四肢,体幹に圧痛を伴う紅色結節,浸潤を触れる紅斑が再発を繰り返した。皮膚生検組織はlobular panniculitisで,脂肪織周囲に組織球,異型リンパ球が浸潤し,胞体に核破片のある組織球も散見された。異型リンパ球はCD3,8,TIA-1,perforin,granzyme B陽性,TCRβ陽性,CD4陰性,CD56陰性,EBER陰性を示し,cytotoxic T cell由来と考えられた。Subcutaneous panniculitis-like T-cell lymphomaと診断してCHOP療法を開始後,解熱し皮下硬結も消失した。本症は発熱,肝障害,汎血球減少を伴い,高率に血球貪食症候群を起こす皮膚原発T細胞リンパ腫である。結節性紅斑様皮疹において難治例,血球減少を伴う例,再発を繰り返す例では本症を念頭におく必要があると考えられる。
