We report a case of rare congenital anomaly of quadricuspid aortic valve and coexisting right coronary displacement. A 70-year-old man was admitted to our hospital for heart murmur and ECG abnormality. The trans-esophageal echocardiography and aortography showed moderate aortic stenosis and regurgitation. At the operation, a quadricuspid aortic valve with three equal cusps and one smaller cusp that showed marked calcification and displacement of right coronary orifice were noted.
