Quality in health can not be reached by sanctions or checks--as experiences show also in other fields. Improvement can be achieved only by a quality philosophy. Qualified and international used tools are the ISO 9000 and EFQM model, even if the application in the health sector requires adaptations.
The German health care system lacks transparency of efficiency and costs. Fixed compensations for common diagnoses and treatments were established to lower spending and to get better statistics of diagnoses and treatments. It could be demonstrated that the expenses are too high while the benefit is too small.
