Four separate groups of patients have been studied: (1) The effect of high-dose methotrexate (MTX) administration on glomerular filtration rate was determined by pre- and posttreatment inulin and creatinine clearances in nine patients. Measurements were made prior to and 24-40 hr after drug administration. Inulin and creatinine clearances both decreased a… (More)
We enrolled 116 patients in a multicenter, randomized, double-blind controlled trial of an 8-week course of 11 plasma exchange (PE) treatments in exacerbations of MS. The control group received sham PE, and both groups received identical treatment with IM ACTH and oral cyclophosphamide. Serum IgG decreased in the PE and sham treatment groups by 76% versus… (More)
Product identification and kinetic data are presented for the conversion of 7,8-dihydrobiopterin into tetrahydrobiopterin by purified rabbit brain dihydrofolate reductase.
Iron overload from repeated transfusions of RBCs in long-term hemodialysis patients is a problem of increasing clinical significance. We report on the prevalence of and diagnostic criteria for identification of hemodialysis patients with iron overload. In 150 unselected hemodialysis patients, 62 (41%) had ferritin levels greater than 2,000 ng/mL (normal =… (More)
The procedures for intensive plasma exchange (IPE) and cytopheresis (CY) require modification for use in children. During the past 11 years we have performed more than 2,000 procedures on children for 11 months to 18 years of age. Heparinization has been our primary method of anticoagulation. Those who were acutely ill anemic or small required a red… (More)
Ten primary and two metastatic central-nervous system (C.N.S.) tumours were assayed. They all contained dihydrofolate reductase. This finding provides a biochemical rationale for antifolate therapy of C.N.S. tumours by methotrexate.
Standard apheresis techniques require modification of use in children, particularly those with serious concurrent medical problems, as they are prone to apheresis-induced disturbances of volume, metabolism, and coagulation. We report 112 plasma exchanges (TPE) on 11 children, 9 of whom weighed less than 20 kg and 7 of whom were critically ill. All were… (More)
