We report a case of xanthogranulomatous pyelonephritis in a cadaver kidney allograft. The patient had diabetic glomerulosclerosis. Simultaneous kidney and pancreas transplantation is the accepted method of treatment in a selected group of type I diabetics with end-stage renal disease. In New Jersey, approximately 60 patients per year fall into this category and the number of patients is expected to increase rapidly.
Air embolization during machine preservation of kidneys for transplantation occurred in seven of 218 kidneys. In three it occured during transportation on a portable machine. Four HIV-infected patients received cadaver kidney transplants. A fifth kidney transplant patient acquired infection through a blood transfusion. The course and prognosis of these patients with HIV infection are described and a review of the literature is presented.
