The last 10 years have seen great activity in the investigation of cerebral catecholamines, particular attention having been paid to dopamine, but new approaches to the therapy have been tried and the sum total of these observations is to challenge peace of mind regarding a seemingly logical chain of events.

The author proposes a novel hypothesis which envisions a variety of transient "trigger factors" acting at the dopamine synapse to increase dopamine turnover, which favours the production of large quantities of free radicals within the cell bodies in the substantia nigra, eventually overflowing the scavenging capacity of neuromelanin and their protective barrier, and causing cell death.

Two new genetic subgroups emerged: a familial metabolic akineto-rigid syndrome (with hypertension, familial diabetes, hypothyroidism and a more severe course) and a familial essential tremor-related Parkinsonian syndrome ( with familial grey hair trait and hyperthyroidism).

It is demonstrated that the prevalence of Parkinson's disease is of uneven distribution within rural areas and the characteristics of the regions of high prevalence were also the areas with the highest use of pesticides.

A new syndrome of autosomal recessive spastic ataxia has been isolated in the Charlevoix-Saguenay region of Quebec and includes spasticity, dysarthria, distal muscle wasting, foot deformities, truncal ataxIA, absence of sensory evoked potentials in the lower limbs, retinal striation reminiscent of early Leber's atrophy.
